If you are injured in a covered accident and the covered injury from such accident causes death within 365 days from the date of the accident, the insurance company will pay the amount shown. On a Family Plan, spouse is covered at 50% of primary benefit and children are covered at 25% of primary benefit.



Blanket Group Sickness Plan & Accident Plan Insurance Benefits

Coverage is accessible through two Blanket Group Policies issued to the UCA.
  1. A Blanket Group Sickness Plan
  2. A Blanket Group Accident Plan
Both plans are underwritten by either Freedom Life Insurance Company of America or National Foundation Life Insurance Company, depending upon your state of residence. Not available in all states

Each Blanket Group Sickness & Accident Insurance Policy provides a basic level of benefit for individuals (and families) that do not have access to traditional coverage. This is a supplement to an essential health benefits plan, under which you must first satisfy a deductible every year before you are eligible to receive benefit payments.* This pays in addition to your coverage under an essential health benefits plan.



*The individual mandate under the Affordable Care Act (“ACA”) generally requires individuals to maintain “minimum essential coverage” in 2014 and beyond, or be subject to payment of the annual shared responsibility payment, the amount of which is based, in part, upon the individual’s household income each year (See paragraph below for details). The UCA Freedom Series Membership plans provide insurance benefits as part of membership, which provide benefits on a periodic basis (e.g. daily/monthly) for covered services and are neither “essential health benefit plans” under the ACA, traditional major medical insurance plans, nor Workers Compensation plans under state law. These plans are “excepted benefit plans” under the ACA, but are not considered “minimum essential coverage” under it. Therefore, unless an insured under one of the UCA Freedom Series Membership plans has an exemption from the ACA’s individual mandate or maintains “minimum essential coverage” under the ACA the insured will be subject to the ACA’s “shared responsibility payment” (See paragraph below for details).

ACA Individual Mandate and Shared Responsibility Payment The individual mandate under the ACA generally requires individuals to have “minimum essential coverage” in 2014 and beyond, or be subject to payment of an annual “shared responsibility payment”, the amount of which is based, in part, upon the individual’s household income each year. The ACA’s “shared responsibility payment” has also been referred to from time to time as a tax and as a penalty, and is payable to the federal government. Specified Disease and Accident plans are exempt from the coverage and rating mandates of the ACA, and therefore are not considered “minimum essential coverage” under the ACA. If an individual (a) does not receive an ACA exemption annually from the federal government for the individual mandate, or (b) does not maintain “minimum essential coverage” under the ACA for 9 or more consecutive months during each year, (including coverage under one of the following types of plans (i) an employer sponsored group health plan, (ii) a grandfathered health plan, (iii) a non-grandfathered health plan for which the government has granted a waiver of the individual mandate, or (iv) an ACA essential health benefits plan), he will be subject to the ACA’s annual “shared responsibility payment”, even if covered under one of the plans. ALSO, THE BENEFITS PROVIDED BY THE POLICIES INCLUDED IN THE UCA MEMBERSHIP CANNOT BE COORDINATED WITH THE BENEFITS PROVIDED BY OTHER COVERAGE. PLEASE REVIEW THE BENEFITS PROVIDED IN THE UCA MEMBERSHIP CAREFULLY TO AVOID A DUPLICATION OF COVERAGE. THE POLICIES INCLUDED IN THE UCA MEMBERSHIP ARE NOT MAJOR MEDICAL OR COMPREHENSIVE MEDICAL INSURANCE COVERAGE, AND ARE NEITHER A MINIMUM ESSENTIAL HEALTH BENEFIT PLAN NOR WORKERS’ COMPENSATION INSURANCE UNDER FEDERAL AND/OR STATE LAW.

The Discount Medical Plans included in membership are provided by Patriot Health Florida, Inc., a discount medical plan organization. The features are not health insurance policies and are not available in all areas. The features provide discounts to certain health care providers for medical service and do not make payments directly to the providers for medical services. The member is obligated to pay for all health care services but will receive a discount from those health care providers who have contracted with Patriot Health Florida, Inc., located at 160 Eileen Way, Syosset, New York 11791, 800-292-3797. Discount medical Plans are not available in AK, MN, MT, ND, SD, VT and WA.
